Abstract

Cardiovascular diseases such as coronary artery disease are the most important causes of death in the world and Iran as well. For this reason, These kinds of diseases cause anxiety on the patietns. Discharge planning is crucial to ensure timely discharge, and continuity of care. It also helps healthcare providers to use limited resources most effectively, and to avoid unnecessary readmissions. The aim of this study is to determie the effect of IDEAL (Include ،Discuss ،Educate ، Assess ،Listen) discharge plan model on anxiety levels in patients with myocardial infarction in Shahid Madai hospital in Khoy, Iran, 2016. Materials and Methods: In a semi experimental study, 108 patients with myocardial infarction were assessed in Shahid Madani hospital affiliated with Urmia University of Medical Sciences in 2016. They were selected through convenience sampling, and divided randomly into two groups of intervention and control. Every patient in the intervention group received IDEAL discharge plan model from their admission to 3 months after their discharge, while patients in the control group received no intervention. Spielberg’s state-trait inventory was used to assess their level of anxiety before intervention, and 3 months after their discharge. Results: According to findings, the mean score of anxiety for experimental group before the intervention and after intervention were 72.71 ± 3.09 and 32.25±4.94 respectively. There was a significant difference between the mean scores of anxiety reduction in two groups after the intervention (p<0.001). Conclusion: As a result, providing the patients with IDEAL discharge plan mode will diminish their anxiety level of myocardial infarction. Therefore, it is recommended that the nurse managers provide programs to highlight the instructional role of nurses
